Tenting of the diaphragm are mostly associated with pseudo-adhesions which is purely physical phenomenon. It is commonly due to acute and chronic respiratory infections. There is no any specific treatment for that. If there is underlying pathology it should be treated.

Physiological or pathological? Some folks are simply put together that way. Not all require intervention.
